Deck 14: Health Manpower Shortages and Surpluses: Definitions, measurement, and Policies
1
Suppose a student is trying to decide if they should become a physician or an accountant. As an accountant, the student would earn $75,000 after four years of education. On the other hand,if the student decides to become a physician, they would need to attend school for 11 years. Each year of school costs $20,000 in both professions. As a physician, the student would earn $100,000.In both cases, the student only has 20 years to work and/or attend school. What should they do?
For the accountant,we need to complete the following:
4 years of school = 4*20,000 = 80,000
The accounting student can work for (20 - 4 =)16 years. So the income earned is
16*75,000 = 1,200,000
Net benefit is $1,200,000 - $80,000 = $1,120,000
For the physician, the cost of schooling is
11*20,000 = $220,000.
The physician can only work for (20 - 11 =)9 years
So the income earned is 9*100,000 = $900,000
Net benefit is $900,000 - $220,000 = $680,000
2
Which of the following is a normative statement?

A)Nonprofit hospitals are inefficient
B)Physicians should be more cost conscious
C)Government-sponsored health insurance will provide lower premiums
D)All of the above
B
A normative statement is a statement of opinion, but a positive (even a false one)is a statement of fact that can be tested.
3
Explain how the current market for residencies is anticompetitive and how market competition would correct the current situation.
The residency market, however,is not a competitive market. In addition to the distorting effects of Medicare's FFS payment system that rewards procedure-oriented specialties more highly and the economic interest of hospitals in their preference for specialist residencies, the number and types of residencies are limited. The most sought-after residency positions have not expanded in response to increased demands. Many medical students have been unsuccessful in entering specialty residencies with high rates of return. It is likely that to maintain high rates of return in certain specialties, residency review committees will have to limit the number of specialty residencies available to hospitals. The continued specialists' high rates of return are likely the result of entry barriers and limits on residency positions in those specialties.
Until a competitive market in residencies can be achieved, the number of residency positions and their distribution will be influenced by the economic interests of hospitals and the different physician specialties.
