Deck 53: Infections of the Eyes, Ears, and Sinuses
1
What is the proper specimen for culture and identification of the agent of acute sinusitis?

A)Aspirated material that has been collected by puncture of the maxillary sinus
B)Drainage material found in the upper respiratory passages that has been collected with a swab
C)Coughed up sinus drainage collected in a sterile cup
D)All of the above are correct.
A
In most cases, a diagnosis can be made on the basis of physical findings, history, radiograph studies, and other imaging techniques such as magnetic resonance imaging. However, if a laboratory diagnosis is needed, an otolaryngologist obtains the material from the maxillary sinus by puncture and aspiration or during surgery. Sinus drainage is unacceptable for smear or culture because this material will be contaminated with aerobic and anaerobic normal respiratory flora.

a.An acute infection of the orbital contents most often caused by bacteria
b.Inflammation of the lacrimal canal
c.Infection of the lacrimal gland characterized by pain of the upper eyelid with erythema
d.Infection of the cornea
Blepharitis
B
Bacteria, viruses, and occasionally lice can cause blepharitis. Although occasionally isolated from surfaces surrounding the healthy eye, Staphylococcus aureus and S. epidermidis are the most common infectious cause of blepharitis in developed countries.

Conjunctivitis in children
A
In children, the most common causes of bacterial conjunctivitis are Haemophilus influenzae, S. pneumoniae, and perhaps S. aureus. S. pneumoniae and H. influenzae (especially aegyptius subsp.) have been responsible for epidemics of conjunctivitis.
