Pericarditis can produce a number of changes in the ECG. Because the ST elevation is related to diffuse patches of inflammation around the pericardium, and not due to an occluded coronary artery, ST elevation is usually diffuse and not strictly grouped into leads that are anatomically contiguous. ST elevation is usually present in most of the chest leads and also in leads I, aVL, II, and aVF. ST depression is often seen in lead aVR. Pericarditis can also produce PR-segment depression. Another change that pericarditis may bring to the ECG is a notching of the J-point. Although not exclusive to pericarditis, J-point notching signifies the possibility of a noninfarct cause of ST elevation.
