Maria and her husband, Trey, have been planning a family for many years. Now that the time seem right, she cannot seem to get pregnant. After two years of trying,they have decided to consult their family physician for a referral to a fertility specialist. Before making the referral, their family doctor runs some tests. Here are the results: Trey is producing sperm at normal levels, and the sperm are healthy and mobile. Maria's hormone levels and all other blood tests are normal. She has a more or less regular menstrual cycle, but she has always had cramps during the middle of her cycle. On further questioning, she admits to the doctor that in the last few years, the pain seemed to get worse, but she thought that was just normal for her. She also admits to some low back pain and some urinary disturbances, particularly around the time of her period.
What should Maria's doctor be looking for, given the timing of the pain during the cycle and the inability to get pregnant?
