التوضيح
A. An electronic health record (EHR) captures the health information of an individual in an electronic format. In addition, the EHR is able to interact with other external healthcare computer programs. This is beneficial and enables healthcare works to perform the following:
1. Transmit prescriptions to pharmacies automatically, using electronic clearinghouses.
2. Transmit and receive medical and lab orders as well as test results, electronically.
3. Compile comprehensive reports, including those of specific diseases and diagnoses that can be shared with government healthcare agencies.
B. There are three EHR models, which include the facility-based model, distribution-based model, and web based personal healthcare model.
A distribution-based model is used in countries operating under a universal healthcare system. This type of model allows for the capture, storage and transmission of healthcare data. Certain information can be electronically transmitted and shared among parties in the healthcare system, with proper consent.
In a facility-based model, the patient data remains in the electronic health system at the facility they visit. Individual healthcare data and reports are not made available to a central government or public health authority.
A web based personal healthcare model allows patients access to a secure web portal for entering and maintaining their personal health data. This information is made available to providers that have access to the proper credentials. The use of these patient portals can also be accessed and updated by providers and physicians, given they have proper access rights.
