Patient Situation
Recall Evelyn (Meet Your Patient). Evelyn is 87 years old and has lived in a nursing home for the past 5 years. She suffers from CHF, hypertension, diabetes, macular degeneration resulting in near blindness, a severe hearing deficit, urinary incontinence, and immobility resulting from a hip fracture. Despite these limitations, she keeps current in the lives of all of her family members and friends. At the nursing home, staff members confide in Evelyn and she knows about their children, their romances, and the gossip around the nursing home. Whenever there is an election, she makes sure that she gets to vote. She "reads" every audio book she can get her hands on.
Recently Evelyn was admitted to the hospital in severe CHI. When leaving the nursing home, she told her pastor, "I don't want to die yet. I'm having too much fun!" You notice that the papers from the nursing home do not contain an advance directive, and it was not mentioned in their report to you.

Nursing Process (Nursing Diagnosis): On the care plan from the nursing home, there were two nursing diagnoses:
• Urge Urinary Incontinence related to difficulty ambulating to bathroom
• Impaired Skin Integrity related to immobility and incontinence Which of these can you remove from the care plan for now? Explain your decision.
