Patient Situation
Marie is 84 years old and has had a right-sided cerebrovascular accident (CVA, or stroke). Doctors say she is unlikely to regain much physical function, if she even awakens from the coma. She was alert and active before her stroke and was able to maintain her independence with the help of a home health aide, who assisted her with activities of daily living (ADLs) and meal preparation three times a week. Kelly and Dan, her children, have met with the physician, who tells them that Marie's chance of survival is slim. He wants to put a feeding tube into Marie and has asked whether they want Marie to be resuscitated in case of cardiac or respiratory arrest. Kelly wants to do what is best for her mother, but she feels that Marie would not want to be kept alive in the condition she is in. She is also concerned about putting her through uncomfortable procedures if there is no chance of recovery. Her brother, Dan, thinks Marie should have the feeding tube. Kelly asks to speak with you and wants to know the best thing to do for her mother.
Thinking
Critical Thinking (Problem-Solving): At this stage of your education, you probably need more information about CVAs and tube feedings. Write a PICO question that you could use to search the literature for information about these two topics and how they are related. See Chapter 8 if you need to Review Pico Questions.
