Forty-two years ago, Dr. Clyde Sorrel opened his medical practice in the town of Clinton, Oklahoma. He was retiring and wanted to sell his practice. Despite having a population just under 10,000, Clinton's diversity index was second only to Oklahoma City, the state capital. Dr. Sorrel ultimately decided to sell the practice to Dr. Janet Livens. Although Dr. Livens was not a "local," she seemed the most interested in the people of Clinton and clearly appreciated the diverse population of the small town. Understanding that news travels fast in small communities, Dr. Sorrel suggested they contact the local newspaper and set up an interview to introduce Dr. Livens to the community. In order to support and reassure and help to develop trust with the people of Clinton, what should Dr. Livens first consider before creating a communication plan?
A) Order an updated demographic study to prepare for the various cultures represented in Clinton.
B) Offer to speak at a Clinton Chamber of Commerce meeting to get a feel for the town.
C) Seek to understand how information is clustered and how people network.
D) Plan an open house inviting citizens through Twitter, e-vites, and a Facebook event.

After analyzing 65 attributes of 80 different countries, on January 23, 2018, U.S. News and World Report published their annual "25 Best Countries in the World" report, and the United States did not make the top 7. While ranking number one in Power, number three in Cultural Influence and Entrepreneurship, the so-called "Greatest nation on earth" is currently ranked number 17 in the Quality of Life Category. One of the Quality of Life attributes, a well-developed public health system, scored a mere 3.7 out of a possible 10 points, leaders in the health-care industry are calling for health-care professionals and leaders to collaborate on how to improve the current public health-care system. As the work begins, it becomes clear that the organizational, social, and health-care cultures within the public health-care system are an issue. Which of the following options supports this statement? (Select all that apply.)
A) Hospitals and community health agencies are slow to incorporate transcultural nursing principles due to a lack of funding despite the repeated requests from health-care staff.
B) The rate of horizontal violence among public health-care workers of different cultural backgrounds has increased.
C) Class-action lawsuits against pharmaceutical companies have become more common.
D) The balance of power has shifted from physicians to nurses due to the close nature of nurse-patient relationship.

As a result of the discussions surrounding the state of the current U.S. public health system, it became clear that transcultural caring is the promise of modern nursing and would be the transformative change to bring about the improvement of how the U.S. public health system delivers care going forward. In order to ensure information about the initiative was in alignment with the newly determined transcultural caring model, all information, guidelines, and training must incorporate the principles of transcultural communicative spiritual-ethical caring. As the Director of Professional Education at Syracuse Center for Health, you create a checklist to use when evaluating potential in-service topics to ensure compliance with the changes. What should be included on the checklist? (Select all that apply.)

Oakville Medical Center's recent visit by the joint commission revealed a deficit in cultural competence amongst health-care providers. Hospital administrators questioned the findings, citing a variety of policies and procedures that clearly outlined the hospitals requirements of their workforce that are specific to cultural competence. The site inspectors noted that after the point of admission, there was a lack of effective communication and an apparent lack of cultural competence. Further, it was reported that rather than focusing on patient and family-centered care, Oakland seemed to treat patients using a "standardized, one-approach-fits-all" approach. Oakland Medical Center was given time to make a plan of correction before a final site report was complete. Administrators created a five-member work group to review the report findings, determine where the center should focus their interventions, and provide recommendations for the plan of correction. When the five work-group members met, they immediately saw that while they all agreed the breakdown in cultural competency initially occurs at the time of assessment, each had a different perspective on the approach. Which of the following options should be included in the plan of correction? (Select all that apply.)
A) Beth proposed that since assessment relied on accurate information, nursing staff should receive specific training on addressing patient/family communication needs.
B) Alex believed that Oakland staff should describe their role in the patient's care during introductions rather than quickly mentioning a name and title.
C) Rita pointed out that sometimes patients come from countries with fewer rights than in the U.S. health-care system, so they should get a patient bill of rights right away.
D) Angela suggested that the plan of correction include clear protocols that provide communication assistance while treatment is being provided.
E) Inez recommended the workgroup include an increased emphasis on understanding patient perspectives as they relate to health care, then including their perspectives when creating treatment plans.
