On October 19, 2007, 19-year-old Ashley Smith was found unconscious in her segregation cell at the Grand Valley Institution for Women and died later that day. The official cause of death was self-initiated asphyxiation (suicide). However, a Correctional Service of Canada report concluded that her death was accidental and the result of a desperate attempt for attention and interaction after she had been confined to isolation for many months.
