1. Edit the case for Hiro Tanaka.
2. Make the Condition tab active.
3. Enter the date of the injury in the Injury/Illness/LMP Date box.
4. Leave the Illness Indicator box blank.
5. In the First Consultation Date box, enter the date Tanaka first saw Dr. Yan for this condition. Press Tab. The program displays a Confirm message stating that the date entered is in the future, and asking whether you want to change it. Click No.
6. Since this visit resulted from a non-work-related accident, leave the Date Similar Symptoms box, the Same/Similar Symptoms box, and the Employment Related box blank.
7. Since this was an emergency visit, place a check mark in the Emergency box by clicking it.
8. Choose Auto in the Accident-Related To box.
9. In the Accident-State box, enter the two-letter abbreviation for the state in which the accident occurred.
10. Tanaka was injured while driving home from a softball game. Complete the Accident-Nature Of box regarding the type of accident with Injured during recreation.
11. Enter the dates Tanaka was unable to work in the Dates- Unable to Work boxes.
12. Enter the dates Tanaka was totally disabled in the Dates-Total Disability boxes.
13. Enter the dates Tanaka was partially disabled in the Dates-Partial Disability boxes.
14. Enter the dates Tanaka was hospitalized in the Dates- Hospitalization boxes.
15. Leave the remaining fields blank.
16. Check your work for accuracy.
17. Save the changes.
18. Do not close the Patient List dialog box.
