On August 11, 1995, David Lyall was adding chlorine to his swimming pool in the backyard of his home. The chlorine was in a tablet form called Leslie's Chlorinator Tablets 1. The tablets were contained in a 5.5-gallon bucket-like container with a childproof lid. Plastican, Inc., was the designer of the container.
When Mr. Lyall attempted to open the container, the lid blew off, allegedly due to the buildup of nitrogen trichloride gas. The lid, chlorine, and chlorine gas blew into his face, causing him to lose his right eye and suffer several chemical burns and cuts to his face, which resulted in scarring and permanent disfigurement. Mr. Lyall's right eye has been replaced with a prosthetic device.
