Aquamarine Company, a manufacturer of heavy machinery, uses materials requisition forms to control the release of raw materials into production. Four copies of a materials requisition form are prepared by personnel in the production department. The supervisor in the production department must authorize cadi materials requisition before It can be used to withdraw materials from the stores department. If the supervisor docs not authorize the requisition, all four copies are marked "not authorized" and filed in a paper file by number. If the supervisor authorizes the requisition, all four copies are taken to the stores department.
