However, this year his wife has insisted that he have a checkup.  Amos arrives at the employee health clinic for the physical exam. On arrival he is visibly anxious and has accepted three calls on his cell phone since entering the clinic. During the initial health history session, Amos reveals that he smokes one pack of cigarettes per day and does not drink alcohol. He states that he has no time for exercise and has gained weight during the past year. Most nights Amos sleeps 4 to 6 hours. He states that he frequently wakes up during the night with thoughts of what he needs to do the next day. He finds it difficult to return to sleep after waking. His past medical and family history indicate that both of his parents died within the past 10 years. His father died of a stroke and his mother died of a heart attack. Amos states that he has been prescribed prazosin (Minipress) in the past but he stopped taking it. When questioned about why he chose not to take the medication, Amos reluctantly confides in you that he suspected that the medication was causing sexual adverse effects.
