Ryan Swabbs, MSN, works at a drug rehabilitation center and provides individual and group counseling for clients who are in the process of controlling or stopping their drug addiction. Tonya Lamburg is a 16-year-old mother of a 2-year-old son who currently lives with his grandmother. Ms. Lamburg entered the drug rehabilitation center with the goal of ending her problem with the use of alcohol and cocaine. Mr. Swabbs is assigned to her case.
At their first meeting, Mr. Swabbs assesses Ms. Lamburg's level of drug abuse and readiness for change. Ms. Lamburg has been at the center for 1 week and has not used any drugs since checking in. She said that she has repeatedly tried to quit alcohol and cocaine "cold turkey" but started to feel "bad and shaky" and went back to using to stop the withdrawal symptoms. "I have no money. I cannot pay for food for my baby. Everything goes to pay for booze or to get high," said Ms. Lamburg. "I dropped out of school when I got pregnant. Everywhere I try to work I get fired. I decided to get help when I saw my baby get into my coke stash. I do not want my boy to die. I do not want to die."
