Exercise 9
Read about the following studies, and then answer the questions at the end of the box.
Standing,T., Anthony, M., Hertz, J. (2001). Nurses' narratives of outcomes after delegation to unlicensed assistive personnel. Outcomes Management for Nursing Practice, 5(1), 18-23.
The authors of this qualitative study analyzed questionnaires and open-ended responses from 148 licensed nurses. Nurses were asked to write two narratives: one to describe a situation in which delegation of a task to a NAP had a positive outcome, and another that had a negative outcome. In addition, phone interviews were conducted with 27 nurses to expand on those findings. Along with the positive reports, the following negative results were also reported:
Negative outcomes occurred most often because the NAP (1) did not follow directions or agency procedures, or (2) was overconfident (e.g., carried out activities that had not been delegated).
With regard to the Five Rights of Delegation, deficiencies were most often related to the right communication or right supervision.
Standing, T., Anthony, M. K. (2008). Delegation: What it means to acute care nurses. Applied Nursing Research, 21(1), 8-14.
The authors of this qualitative study conducted 17 in-depth interviews with acute-care nurses, both novice and experienced, to examine the nature of delegation.
Nurses expressed frustration that they were being held accountable for the NAPs' tasks, while the NAPs themselves were not. Several nurses described NAPs' resentment of RNs. They indicated that the RN-NAP relationship and good communication are at the heart of delegation. The RNs recognized the importance of acknowledging the contribution of the NAP. The RNs pointed out that one negative aspect of delegation is that the NAP sometimes did not perform tasks well, or at all, and sometimes communicated improperly with patients. A positive aspect is that delegation allows the RN time to monitor patient changes, review charts, and focus on more important things. They concluded that nursing education should provide more content on communication and interpersonal relationships.
What similarities do you see between the two studies in the authors and the methods?
