Patient Situation
Mrs. Vasquez, a 70-year-old Mexican American woman, has diabetes mellitus and has come to the hospital because of a sore on her lower leg that will not heal. Mrs. Vasquez, a widow, cares for herself at home. She speaks only broken English and has some difficulty understanding your questions. After great effort on your part, you determine that she has missed her last appointment with her diabetes specialist because she has no transportation, so she has just been putting a dressing on the sore. Mrs. Vasquez tells you that she loves to see her grandchildren, who visit often, and that she enjoys cooking and eating Mexican food. She also says that she believes she is sick because she has not pleased God. However, God is a source of com-fort and assurance to her. She prays a lot and often has rosary beads in her hand.
Doing
Nursing Process (Analysis/Diagnosis): Which nursing diagnosis do you think would be best for planning Mrs. Vasquez's care? (Recall that the etiology should suggest your nursing interventions and the diagnostic label should suggest the expected patient outcome.) Explain your reasoning.
• Noncompliance (with clinic appointments) r/t lack of transportation
• Impaired Skin Integrity r/t self-treatment of ulcer
• Noncompliance (with clinic appointments) r/t beliefs about God and illness
