CASE A
This 54-year-old female is undergoing orthodontic therapy in response to temporomandibular disorder (TMD). The maxillary orthodontic appliances were placed 18 months ago after an examination revealed various issues with malocclusion that were contributing to her chief complaint of temporomandibular joint (TMJ) pain and "sore jaws." (See Figure 6-31.)

Overlapping of adjacent teeth contacts that is considered acceptable would indicate that the area of interest was an object that is positioned very far posterior in the oral cavity. Overlapping would only be acceptable for a molar periapical radiograph. It is evident in this maxillary left molar periapical radiograph that the object of interest is a third molar. It appears that the only option to being able to record the third molar in its entirety was to apply the disto-oblique technique. A shift in horizontal and vertical angulations allows the radiographer to angle the x-ray beam such that the posterior object is projected forward onto the image receptor. The disto-oblique technique results in an increase in horizontal overlap , possibly not record ing the entire occlusal edges of all teeth on the image. This is considered acceptable if the goal was to record this third molar.
