It takes around 4-5 days for the levels of these factors to decrease enough to achieve a therapeutic effect with Coumadin alone. During this time, heparin is given to provide immediate anticoagulation until Coumadin takes effect. Once Coumadin has achieved its full effect, heparin can be discontinued.
