In both effective and ineffective erythropoiesis,the rate of red cell production in the bone marrow is increased (increased erythropoiesis).However,release to circulation differs in the two: it is successful in effective but not in ineffective,where significant intramedullary loss occurs.Thus the RPI is greater than 3 in effective,but usually less than 2 in ineffective.Red cell survival in circulation is significantly decreased in effective,but essentially normal or insignificantly shortened in ineffective.
