While working in the emergency room, you meet a young couple, Brittany and Colby. They are currently on their honeymoon, and Brittany is not feeling well. She reports burning upon urination, and a low-grade fever. They assumed she had contracted a urinary tract infection, and had been treating it with cranberry juice for 2 days, following the advice they had received from Brittany’s mom. However, symptoms have not improved. They are concerned because she now has the frequent urge to urinate, and she is developing pain at her bladder. Her low-grade fever persists.

The presence of high fever and low back pain may indicate a more complicated diagnosis, such as pyelonephritis (infection of the kidneys). This would require further evaluation and potentially different treatment than a simple urinary tract infection. Lack of fever and RLQ (right lower quadrant) tenderness may indicate a different condition, such as appendicitis. Nausea and vomiting are common symptoms of many conditions and may or may not be concerning in this case. Lack of pain and pink urine do not provide any indication of a more complicated diagnosis.
