Most people with early periodontitis respond well to periodontal debridement. Oral prophylaxis is reserved for individuals with gingivitis, not periodontitis. Gingival curettage, in nonsurgical periodontal therapy, is no longer considered to be an acceptable intervention due to evidence indicating scaling and root planing alone has similar results to scaling and root planing with gingival (soft-tissue) curettage. (Refer to the AAP statement about gingival curettage.) Gingivectomy is a surgical intervention used for gingival enlargement because it is a surgical therapy. It is used for pocket reduction by complete removal of the soft-tissue pocket wall. Root planing is intentional removal of cementum and endotoxin to promote healing. Used alone it refers to a technique no longer recommended in NSPT. Now removal of cementum only occurs unintentionally when calculus is embedded in the cementum, or in an area where healing has not taken place. Research has demonstrated that endotoxin removal is accomplished by periodontal debridement (therapeutic scaling and root paling).
