The presence of furcations alone does not indicate consideration for periodontal surgery. Presence of furcation involvement does indicate moderate or advanced disease severity and compromises the health of the tooth or teeth. Some furcations, however, are minimal and if the client is instructed to care for this area appropriately and adheres to PM, then these types of furcations (Class I) can be prevented from advancing. In some cases, access to the furcation itself might be present, allowing the clinician and client to adequately care for the furcation. If it is suspected that the periodontal debridement is not effective due to pocket depth, pocket topography, and other complicating factors, then referral for surgery is considered. If healing of deep pockets is not achieved, especially after PM, then periodontal surgery is considered. Osseous defects are considered for surgical intervention in moderate and advanced severity. If the clinical attachment level is stable, as identified over PM visits, this status might be adequate for healing, especially depending on the disease severity (i.e., early disease state or other disease state where surgery is not indicated and the periodontium appears to be healthy).
