Serious insult to irradiated mucosal tissue and bone can be caused from infection and traumatic injury. Dental prophylaxis will not precipitate necrosis and is, in fact, very important in maintaining the integrity of the periodontal structures following radiation therapy. Periodontal infection, by definition, has caused damage to the connective tissue and bone. Therefore, any damage to the bone can result in osteoradionecrosis. A denture irritation/ulcer, if severe enough, has been known to compromise underlying tissue and bone possibly leading to osteoradionecrosis. Oral surgery, especially those performed in the site radiated, runs a high risk of osteoradionecrosis.
