Perimylolysis is difficult for practitioners to identify because initial tooth structure loss usually is subtle. Slight pitting on the incisal surfaces of the anterior teeth and a cupping appearance on the cusps of the posterior teeth are usually the erosive changes that are the first to be detected in the client with frequent vomiting behavior. As perimylolysis progresses, the teeth exhibit a loss of normal anatomic features, such as developmental grooves and pits, and they develop a matte like surface (thus teeth do not have a glossy appearance) with rounded margins. This may become so extensive that a complete loss of enamel is evident. Loss of enamel around amalgam restorations results in a raised island appearance of the amalgam. Teeth without restorations show a significant loss of occlusal contours.
