Neuroses, according to Adler, are attempts at seeking to excuse oneself from his or her obligations. There is no distinction as to which pieces are included or excluded. The therapist is given an understanding of the neuroses through the client's presentation. While it seems to be a bit callous, Adler gives much personal attachment to disorders and sees them as serving a purpose specifically for the client, imagined or otherwise.
