Sickle cell anemia, a chronic hereditary hemolytic form of anemia, is found predominately in those of the black race. The presence of Hgb S along with Hgb A is noted in erythrocytes, causing them to acquire a sickle or elongated shape on deoxygenation (see Figure 10-54). These rigid, misshapen cells obstruct capillary flow and lead to tissue hypoxia and further sickling; this in turn causes further obstruction and eventually infarction. When the sickling causes an obstruction, the individual experiences severe pain. When the sickled cells are reoxygenated, they resume the natural round disk shape of a normal erythrocyte. In addition, Hgb S has reduced oxygen-carrying capacity.
