Usually insidious in onset, Parkinson's symptoms, which vary from person to person, may be associated with aging until the recognizable paradigm of Parkinson's disease emerges. A primary symptom is tremor or trembling in hands, arms, jaw, face, and legs. In addition, there is a rigidity or stiffness of limbs and trunk, causing the posture to be stooped. The patient moves slowly with a peculiar shuffling gait: the head is bowed, the body is flexed forward, the knees are slightly bent, and the patient has a tendency to fall due to impaired balance and coordination (see Figure 13-18). The face takes on a masklike or expressionless appearance, speech is muffled, and swallowing is difficult. Gradual changes in behavior and mental activity are noted in some patients as the disease progresses. The mean age of onset is 60 years, but many cases appear in younger persons.
