The needs assessment process can be viewed within three general steps that are essential regardless of the population, community, and need or the complexity of programming (Gilmore & Campbell, 2005; Hodges & Videto, 2011; Soriano, 2013). Figure 5-3 denotes the first step: describing the population, which involves gathering data to create an accurate picture of the target population and the need. In this step, it is important to elicit the voices of members of the target population through interviews, focus groups, or surveys to appreciate the complexities of their health needs through the collection of relevant demographic data and the identification of key health issues, behavioral health risk factors, health disparities and values, and beliefs and attitudes about health (Issel & Wells, 2018). As noted by Alley, Asomugha, Conway, and Sanghavi (2016), "In our current health system, patient health-related social needs frequently remain undetected and unaddressed" (p. 8).
