Thiamin deficiency leads to decreased activity of enzymes that require thiamin as a cofactor, such as pyruvate dehydrogenase and α-ketoglutarate dehydrogenase, resulting in increased plasma concentrations of pyruvate and lactate.  Thiamin serves as the precursor for TPP. TPP is necessary as a cofactor for the pyruvate dehydrogenase complex (PDHc) and, therefore, a deficiency thiamin results in impaired oxidation of pyruvate. This will lead to increased plasma concentrations of pyruvate. When lactate from erythrocyte and skeletal muscle metabolism reaches the liver, it will not be readily converted to pyruvate due to the reduced pyruvate oxidative capacity leading to elevated plasma lactate. In addition, the excess pyruvate will be a substrate for lactate dehydrogenase, resulting in additionally increased levels of pyruvate.
