Glucose 6-phosphatase is crucial for the final step of gluconeogenesis and glycogenolysis, converting glucose-6-phosphate to glucose. Its deficiency leads to impaired glucose release from the liver, causing increased hepatic glycogen accumulation.  Deficiencies in glucose 6-phosphatase represent the causes of type 1 glycogen storage disease, more commonly called von Gierke disease. The hallmark features of this disease are hypoglycemia, lactic acidosis, hyperuricemia, and hyperlipidemia. When the liver is unable to release free glucose, it is diverted into excess glycogen storage which leads to hepatomegaly and consequent hepatic dysfunction.
