PM continues for the life of the dentition or its implant replacements, because clients with gingivitis and periodontitis have a disease entity, like other systemic disease, that should be controlled by frequent periodontal care to suppress periodontal pathogens. Self-care also needs to be reinforced. One year after initial therapy does not support the philosophy of continuing periodontal care at frequent intervals to suppress periodontal pathogens. It also does not support frequent reevaluation and reeducation on oral self-care. Likewise, 5 years has no relevance or evidence-based research to support its length. Halting of periodontal attachment loss is one of the factors in determining the therapeutic endpoint of nonsurgical periodontal therapy determined at the reevaluation visit. PM at appropriately timed intervals must continue to prevent recurring periodontal attachment loss. Gain of clinical attachment level is ideal but not always achieved depending on the clinical attachment level to begin with, self-care, risk factor control, or thoroughness of debridement.
