A) provides the patient's demographic information such as name,age,birth date,address,etc.
B) specifies the patient's desires regarding care should he or she become incapacitated and regarding end-of-life care.
C) provides information about the patient's past medical history and current health status.
D) is an agreement between the patient and the hospital that provides a legal consent for treatment.
