The client has an unmet human need for responsibility for oral health due to the client’s impaired physical ability as evidenced by the client verbalizing that it is painful to grip a toothbrush and floss since arthritis in the hands has worsened and the plaque free index score has decreased to 55% since last continued care visit. The dental hygienist, in collaboration with the client, set the following goal: the client will increase the plaque free index score to 85% by 7/12.
