In order for a Root Cause Analysis to be credible it must contain an explanation of all causes, even if it is determined to be "not applicable". Each step of the process under investigation is analyzed and all possible causes are documented even when the group concurs that a possible cause did not contribute to the sentinel event. Some of the other things that must be done in order for this process to be credible include the active participation of leadership, the active involvement of all individuals who are closely involved in the particular process, and a review of relevant literature.
