In order for a Root Cause Analysis to be thorough it must identify all risk points. Processes are complex and, as such, there are risk points. Many of these risk points occur as a result of interdepartmental communication and coordination. Some of the other things that must be done in order for this process to be thorough include an analysis of the process with a series of "Why" questions and one that includes all aspects of JCAHO's Minimum Scope of Review of Root Cause Analysis.
