Even if one was not sure which level of echogenicity was the normal testicular tissue, the presence of two different levels (aside from non-mass-like structures such as the mediastinum testis) is abnormal. As is typical for this case of seminoma, the mass is relatively hypoechoic to the brighter normal testicular tissue. Varicocele and hydrocele are not findings on this image. The bright foci of microlithiasis stand out much more than the occasional bright echo that is shown in this image.
