Type 2 diabetes is characterized by insulin resistance, where there is adequate insulin secretion but impaired response at the cellular level, particularly in muscle, fat, and liver cells.  Unlike patients with T1D, those with T2D have detectable levels of circulating insulin but these individuals are resistant to the peripheral actions of insulin. In the progression from impaired glucose tolerance to full-blown T2D, the level of insulin secretion declines indicating that patients with T2D eventually have decreased insulin secretion. Additional studies have subsequently demonstrated that both insulin resistance and insulin deficiency is common in the average T2D patient.
