Recommendations for protein intake in older adults are based on nitrogen balance studies in young adults and that muscle loss in the older adult, called sarcopenia, is not prevented with the recommended intake of protein because of a blunted response to protein synthesis that occurs with age. The client should meet with the dietitian to discuss protein needs to prevent the development of sarcopenia. An overweight client does not necessarily need assistance with protein needs, nor does an adolescent recovering from pneumonia. The young adult who has a fractured leg will have increased protein needs for healing, but should not need a dietician to assist with planning for that.
